Eligibility determination and service coordination unique to each individual
Any person that has been diagnosed or is suspected to have an intellectual or developmental disability will meet with one of our professional Individual Service Coordinators (ISC) where eligibility for state-funded services will be discussed. Eligibility is obtained through in-depth interviews, compilation of historical documents and comprehensive assessments. Once eligibility has been confirmed, the ISC will assist the individual and their family with exploring the service options that will support and address the individual’s current needs.
